Subacute Skilled Nursing


Subacute skilled nursing, also known as short-term rehab, supports each patient’s recovery. Services include continuing medical care and rehabilitative therapies, such as speech, occupational and physical therapy.

At Eddy nursing home facilities, you receive an exceptional level of care that includes:

  • Easy access to doctors: Our physicians are always available to provide medical expertise and guidance, and they are frequently on-site to see and visit residents. We also communicate regularly with St. Peter’s Health Partners specialists, making it easier to coordinate services for urgent needs.
  • Hands-on approach: Patients receive therapies up to five days a week, which is more than what’s typically available. Receiving more therapy can help speed recovery.
  • Specialized services: Some locations offer advanced treatments, including peritoneal dialysis for advanced kidney disease. Select locations also deliver total parenteral nutrition in people who cannot take food by mouth.

Long-Term Care


Long-term care provides a safe, comfortable environment to individuals who need a helping hand. Many patients are older adults or have advanced neurologic disease, including dementia.

  • Care consistency: Some of our nurses have been taking care of Eddy long-term care patients for decades. Their passion for treating nursing home patients create a unique patient experience.
  • Memory units: Select Eddy locations have Alzheimer’s and dementia units. Specially trained nurses work with therapists to tailor services to the needs of people with memory loss.
  • Innovative Living Concepts: Among the options we offer are home-like environments where residents live in small, single story ranch-style houses with other seniors, where they enjoy meals together seated around a large family-style dining room table.
