We can help

If you live in Albany, Rensselaer, or Schenectady Counties in the Capital Region or Cayuga, Cortland, Madison, Oneida, Onondaga, or Oswego Counties in central New York and are a Medicaid member, our care coordinators can help you obtain a doctor, find transportation to a doctor appointment, help make sure you have what you may need after being in the hospital, and so much more.
