Home-Based Primary Care

Our Home-Based Primary Care program is staffed by a team of nurse practitioners and collaborating physicians who provide the same high-quality primary medical care in patient homes as they do in a traditional medical office. This program is designed for individuals with chronic illnesses who regularly require in-home services or have a physical condition which makes it difficult for them to leave the home. Nurse practitioners are on-call after hours, including weekends and holidays.

The cost of Home-Based Primary Care is the same as an office visit.
