Application Requirements
Minimum Entrance Requirements:
- Applicants must have graduated from an accredited college of pharmacy with a Doctor of Pharmacy degree
- Applicants must have a NYS pharmacist license or be eligible for licensure in NYS
- All matched residents that are not a NYS Licensed Pharmacist prior to starting the residency program MUST obtain a NYS Pharmacy Limited (Intern) Permit and supply a copy to the Residency Program director prior to day one of the residency program
- Applicants must be legally authorized to work in the United States. Due to the one-year duration of the PGY-1 residency experience we are unable to support applicants with Visas
- Applications are only accepted through PhORCAS. Applicants will be asked to submit the following:
- Official transcript from the college
- Curriculum vitae
- Letter of intent (Personal Statement)
- 3 letters of recommendation
- Three writing samples
- Application Deadline: January 6, 2025
- Applicants shall participate in and comply with the rules of the ASHP Residency Matching Program
