If you’re on Medicaid and have certain health problems, it can be difficult to find the services you need.  The Health Home program at St. Peter’s Health Partners can help if you: 

  • Have two or more chronic (long-lasting) health problems
  • Have to go to the emergency room a lot
  • Have trouble getting appointments or can’t get a ride to the doctor
  • Need a safer place to live, healthier foods or medications 

What Is a Health Home? 

A Health Home isn’t an actual place. It’s a group of people who work with you to find ways to keep you healthier. It’s free and available to adults and children on Medicaid who have any of the following:

  • HIV or AIDS
  • Serious mental health illness
  • Two or more long-term health problems, such as diabetes, asthma, high blood pressure, heart disease, overweight/obesity and substance use disorder 

What Can a Health Home Do for Me?

If you choose to enroll in a Health Home, you will be assigned a care manager. Your care manager will find out what you need and organize it for you, such as:

  • Identify the kinds of health care providers you need, including services for mental health issues or substance use disorders (addiction)
  • Make appointments and set up transportation so you can get to appointments 
  • Ensure that all of your health care providers talk to each other so that you get better overall care 
  • Make sure you get the medications you need
  • Help you find better housing or healthier food
  • Teach you ways to live a healthier life
  • Check in with you regularly to see if you’re OK

The Health Home program can help you become healthier, maintain your health and stay out of the hospital. Enrollment is voluntary and you can stop using the program whenever you like.
